Final Grant Report & Survey Guidelines
Overview
Thank you for participating in the Business Expansion and Development Grant Program. As a condition of your grant award, a Final Grant Report and Survey must be submitted within fourteen (14) days following June 30, 2026.
The purpose of this report is to document project outcomes, economic impact, use of grant funds, and lessons learned during the grant period. Information collected may be used by the Division to evaluate program effectiveness, measure community impact, and highlight successful projects and businesses.
Please review all requirements carefully before beginning your submission.
Submission Requirements
All required fields and supporting documentation must be completed and uploaded before submission. Incomplete reports may delay grant closeout or require follow-up documentation.
Required Components
Your submission must include:
A title page and table of contents with the GRANTEE logo, and image of the business such as
improvements made or general photo for media use.
A 250-word summary of the business expansion/development project and its outcomes including the
county, and location of the project.
Media assets of the business, including a single, one to three (1-3) minute video, four (4)
high quality photos, and implied media-release to use the content to highlight your
business, and creative industries division in the future.
A two-paragraph minimum to two-page maximum review of all grant-related activities,
successes, and challenges of the project. (This can be combined with/included in the
overall economic contributions reporting outlined below in point ‘e.’)
Overall economic contributions including:
Revenue growth and financial impact
New business partnerships or collaborations
Increase in employees, contractors, or creatives hired
Audience reach and engagement metrics
Return on Investment (ROI) for grant funds used
Challenges and successes in business expansion, suggestions for future
programming for the division
Number of businesses and entrepreneurs impacted, and their industry area if
applicable i.e.:
1. performing arts | visual arts | literary arts
2. entertainment | media | information and broadcasting
3. applied arts and design, including architecture, landscape architecture,
museum and gallery professions
4. promotion | marketing | graphics and industrial design
5. technology and computer system design | software design | coding and
digital media
6. crafts and artisan professions including metal, wood, glass, ceramics,
paper, printing, textile, and culinary arts
7. other
viii.Final executed budget for full grant of $10,000.00 including invoices, costs, receipts, and matching funds if applicable.
